Catastrophic Illness in Children Fund—(800) 335-3863; financial assistance for children with a catastrophic illness up to 21 years old, must be a resident of NJ (can be undocumented)

Charity Care—1(866) 588-5696; free or reduced charge care for inpatient and outpatient services at acute care hospitals throughout NJ, assistance is available to NJ residents who have no health coverage or coverage that only pays part of the bill, are ineligible for private and governmental sponsored coverage such as Medicaid, and meet income and asset eligibility criteria, call or visit www.state.nj.us/health/charitycare for these criteria

Free Cancer Screenings and Mammograms through VNA of Central Jersey—(732) 933-3952; for men and women between the ages of 18 - 64 (women over the age of 40 for mammograms) who are uninsured, live in Monmouth County, and meet financial eligibility requirements, call to confirm eligibility and schedule an appointment

Health Insurance Exchange—1(800) 318-2596, www.HealthCare.gov; assistance finding health insurance for those who do not have insurance through a job, Medicaid, Medicare, CHIP or other sources that provide qualifying coverage, may also apply for health insurance subsidies if income qualifications are met

Medical Emergency Payment Program for Undocumented Residents—if you are an undocumented resident who receives emergency medical care in New Jersey, ask the hospital staff for an application. If they cannot provide an application, ask the hospital staff to inform Monmouth County Social Services (732) 431-6000 and set up an interview to apply

NJ AIDS/HIV/STD Hotline—(800) 624-2377; information on free testing sites, risk factors, treatment, drug interactions, side effects, and partner notification

NJ FamilyCare—(800) 701-0710; call to receive an application by mail by giving your name and address, applications can also be downloaded or you can apply online at www.NJFamilyCare.org. If you need assistance completing your application, call to speak to a Health Benefits Coordinator or visit an outreach site for assistance with an application.
